Both files use the NHS hazard log layout (DCB 0129 and DCB 0160): Hazard ID, date added, effect, hazard, harm, possible causes, existing and additional controls by design, training and process, initial and residual ratings, justification and risk status.

• The .xlsx has two sheets: “Hazard Log” in the NHS layout, then “Qyro detail” with the theme, parent, affected elements, control categories and approval history.
• The .csv is the “Hazard Log” sheet only.
• Hazard ID is the reference from your imported log where there was one, otherwise HAZ-001 onwards.

Publishing freezes the assessment as a baseline, with its approval history. Work continues in a new draft.
